The Sandwich Public Health Nursing Department will be offering flu shots to the residents of Sandwich, aged 19 and older at a Drive-Thru Flu clinic.


**Fill out your own insurance form by downloading and printing the form by clicking here. Please have form filled in prior to arriving at your appointment. Thank you! **


Please: ** Do not come if you feel sick.  ** Wear a mask.   ** Wear a short sleeved shirt. ** Do not bring pets with you to your appointment.


Most insurances will cover the flu shot. Please bring ALL medical insurance cards including Medicare and MassHealth. We do not need dental, eye, or prescription drug cards.


Adults who will be having surgery, chemotherapy, or radiation treatments prior to the clinic dates should call the office to make an earlier appointment.

The Equipment Shed will be available on Mondays and Fridays, by appointment, for equipment pick-up and drop off. Please call the office to discuss setting up a time to visit the shed. Appointments are required as the office is still currently closed to the general public.

Blood pressure checks have not yet resumed at this time. Check back for further updates.

Overview

The Public Health Nurse’s Office provides the following services and programs to the residents of Sandwich:

  • COVID-19 Information and Contact Tracing
  • Tick Education and Removal (see below for educational video)
  • Mosquito-Borne Illness Prevention Counseling (See our Mosquito page for EEE and WNV updates)
  • Blood Pressure Checks
  • Child and Adult Immunizations (Shingrix is available)
  • Travel Immunizations
  • Communicable Disease Follow-up
  • Durable Medical Equipment Loan Closet (wheelchairs, shower chairs, walkers, canes, commodes, crutches, etc.)
  • Home Visit for Assessments
  • Flu Clinics
  • Medication and Dietary Counseling
  • Suture Removal and Wound Care
