Did you misplace your COVID-19 Vaccination Card? You will need proof of your initial COVID-19 Vaccination if you intend on getting the COVID-19 Booster shot. Maybe you plan on traveling or going to an event at the TD Garden? You will need your vaccination card. If you do not know where your vaccination card is you can obtain a copy of your COVID-19 Vaccination here - https://www.mass.gov/info-details/requesting-a-copy-of-your-covid-19-vaccination-record.
