The Sandwich Public Health Nursing department has scheduled a MODERNA Booster Shot Clinic!
(Booster shots only, please call the office if you need a first or second dose).

Wednesday, December 1st from 9:00am – 2:00pm at 270 Quaker Meetinghouse Road, East Sandwich
- Please do not come to the clinic if you do not feel well.
- Please wear a mask.
- Please wear a short sleeved shirt.
- Please bring your insurance information.
- Please bring your vaccination record card to your appointment. If you do not have a vaccination record card, you can visit mass.gov following your booster shot to obtain a copy of your complete record.
